Rotation:
First and Initial Rune Cooldown
IT - PS - DS - HS - HS - RT - HS

Second Rune Cooldown (and sustained after initial)
DS - HS - HS - HS - HS

This is the base rotation. You will need to use pestilence in place of a Heart Strike when your diseases are about to run out. I time mine optimally for the last second of the disease (good rule of thumb is when your GCD is refreshing and the dot is showing ~2 seconds on it.) I WILL NOT GUARANTEE that this rotation is doable all the time. If your rotation gets messed up, try to have it so you use one Death Strike and 4 Heart Strikes a rotation, especially with the Death Strike using 2 non-death runes. Also use Rune Tap every time it's up at the end of a rotation, bury it in a heart strike.

Before you ask, the 2 points in endless winter give you 2%/4% strength with 3.3.3
For raiding, the points in Icy Talons are possibly not the best idea, so you could go 3 points in Annihilation and the other 2 where ever you like, I would do 1 in Icy Reach and 1 in Lichborne

SS - SS - BS - BS - DC

Rinse and Repeat with First then Second Rotations

Death Coil during gaps, use Rune Tap to add a Blood Strike or Pestilence, etc. With the change to Scourge Strike and the fact that my gear is mostly Armor Penetration based, I went for the Reaping route, which is as follows.

This is my current dps spec, and I'm able to put out 10k on our 25 mans. Rotation wise it's similar to the reapingless sub-frost build, but that build is apparently more for people using proc weapons because of the haste (Bryntoll and Shadowmourne).

First Rotation:
IT-PS-SS-BS-BS-DC

Second Rotation:
SS-SS-SS-DC

Rinse and Repeat. For AoE, pestilence in place of a blood strike. It is imperative that you use at least one blood strike per rotation to keep the 5% damage buff going.
